    Rates

    Individual Clinical Supervision for Social Workers: $85 per hour

    Individual Counseling: $150 per session 

    Collateral Services (Participation in meeting or conference, letter writing, completing forms):  $25 per 15 minutes

    Medical Records Request: $30

    Legal Fees (Testimony, responding to subpoena, all other legal-related work for a client or 3rd party): $200 per hour

    Insurance

    Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy telehealth services.

    I am currently an in-network provider with Anthem Blue Cross Blue Shield, Optum Behavioral Health, UnitedHealthcare, Oxford Health Insurance, Medical Mutual, UMR, Cigna, Aetna, and Oscar Health.  I can also provide you with a superbill if you have out-of-network benefits with your plan.

    When you contact your insurance provider, please provide them with my NPI #1457996381.

    I recommend asking these questions to your insurance provider to help determine your benefits:

    • Does my health insurance plan include online mental health benefits?
    • Do I have a deductible? If so, what is it and have I met it yet?
    • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
    • Do I need written approval from my primary care physician in order for services to be covered?
    • Do I have a copay per session?

    Payment

    I accept all major credit/debit cards and HSA/FSA cards as forms of payment. 

    I am unable to provide a sliding scale fee or accept any EAP plans at this time.

    Cancellation Policy

    If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you will be charged a late cancellation or no-show fee of $75.

    Good Faith Estimate

    You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
